New research on epigenetic sperm quality advances fertility care through science

The study, “Epigenetic sperm quality testing for predicting fertility treatment success: a real-world and multi-site analysis,” represents one of the largest real-world evaluations of epigenetic sperm assessment to date, drawing on data from fertility clinics across the United States.
Moving beyond traditional male fertility metrics
Male fertility evaluation has historically relied on standard semen analysis, assessing sperm concentration, motility and morphology. While foundational, these parameters do not always reflect the underlying biological factors that influence fertilization, embryo development and pregnancy success.
This research focuses on epigenetic sperm quality, which evaluates molecular markers associated with sperm function and developmental competence. These epigenetic patterns provide insight into sperm health that cannot be observed through conventional testing alone.
Real-world findings across multiple clinics
This multi-center analysis evaluated outcomes from more than 500 couples undergoing fertility treatment and revealed several clinically meaningful findings:
Abnormal epigenetic sperm quality was associated with significantly lower pregnancy rates following intrauterine insemination (IUI), even when traditional semen analysis parameters were similar.
IVF with intracytoplasmic sperm injection (ICSI) outcomes were not negatively affected by epigenetic sperm quality, suggesting that ICSI may help overcome certain sperm-related biological limitations.
These results highlight the potential role of epigenetic sperm testing in helping clinicians identify patients who may benefit from earlier transition to more advanced treatment options.
Importantly, the study reflects everyday clinical practice, rather than a tightly controlled trial environment, underscoring the relevance of these findings to real-world fertility care.

The results suggest that incorporating epigenetic sperm assessment, when clinically appropriate, may help fertility specialists:
